The first time you visit the LCS shop for service work, we will ask you to fill out our Customer Profile Form. With the profile form, we gather basic contact information for the person who will be responsible for payment of goods and services.  It is also used to collect information specific to the rider regarding weight, height, riding habits, etc.

Each time you bring in a new project, we’ll ask you to complete the New Project Check In Form. This form is the first step in getting your project in the LCS system. It also includes disclaimers that we want to present to our customers in written format. You are welcome to a signed copy of this form.

New Project Check In Form

There are a few instances where you may ask us to put charges on your credit card (i.e. you are not physically in our shop to present your card and to sign the credit card receipt) that we will ask you to complete the Credit Card Authorization Form. We do not keep credit card information on file, and once this form has been sent to us, and the charge processed, we will disfigure the credit card number so that the complete number is not legible.
